Key Signs of Digital/Org Transformation Failure.

I'm learning about some of the principles of software and organisational change.
My hope is that some of you are willing to contribute a little to the conversation below on the series of steps to recognising the failure, or impending failure, of a transformation.
At what point is the project irretreivable and can the same leader unpick their circumstances and get the programme back on track.
Actual, anonymised, examples are appreciated as are events that correspond to milestones and standards for a particular implementation.

The seeds of transformational failure are sown when the higher-ups attempt to delegate it, as if it were any other strategic decision. This is fine when it's the kind of work the company is already geared up to do, but it doesn't work with change. Change is a hot potato and there is an organizational gravity to overcome, especially when the directive hits the "frozen middle" who are the company's officers. It's up to senior leadership to create, communicate and reinforce a sense of urgency for change so escape velocity is achieved.
