Assuming the bug escaped into a release, what Scrum event do you feel this topic should be discussed? And what Scrum commitment out to be inspected and potentially adapted by the Scrum Team?

We may need more context to answer this. The 'right questions' may not even be needed if the Developers act with accountability and address the issue.

Without more context and information about the team's way of working, I'd say the Scrum Master would observe how the team receives, prioritizes, and resolves the defect. It would be also a good idea to observe how the team works to improve their quality. Depending on the team's way of working, there may be ways for the Scrum Master to help or to facilitate the process.

Perhaps nothing. A call to action would be suggested when bugs are not detected by the team.

In the situation you describe, we may have evidence that the team's techniques, quality checks and assurance of Done are all in fact working. The Developers might be doing test driven development for example, which involves the exposure and resolution of unsatisfactory system behaviors.

I'd suggest that the right question for a Scrum Master to ask might be: am I seeing a problem at all?
