Accountable and responsible

What is the difference between accountable and responsible?

Someone who is responsible for something is the person who is expected or obligated to do that thing. Someone who is accountable is expected to ensure that the work is done and done correctly or must justify or face the consequences if it is not.

The word "accountable" is used once in the 2017 Scrum Guide. The word "responsible" is used 7 times. Is there a particular point of confusion between accountability and responsibility?

Sometimes the people who do the work (i.e. those responsible) are not answerable (i.e. accountable) for the outcome.

I'd suggest that, in agile practice, any such difference ought to be minimized.
