Refined the Nexus

Hello people,

I was reading the Nexus and I had some doubts.

When refining the nexus, do I refine all items in the product backlog at once?

Only the nexus integration team participates?

In planning, do scrum teams take the already refined items separately for them and refine them again? Are these team refinements done during the sprints?

Could someone explain this process to me better?
